Distance Sensor issue

Connecting the distance sensor and LED per Lesson 38 of the Ultimate sensor kit PDF. getting the following error:

IOError: [Errno 121] Remote I/O error

Any help would be greatly appreciated.

Also the i2c interface is enabled and I'm getting back /dev/i2c-1 - all connections seem to be correct.
